Undergoing treatment for a serious illness and your doctor isn’t in the Aetna® network? Included Health can help you.
A Transition of Care (TOC) program is available for members receiving ongoing complex medical care (for certain medical and behavioral health conditions) who are transitioning to a new healthcare company during the new hire period or annual enrollment (for example, moving from Cigna or Kaiser to Aetna).
- A transition of care can include in-network reimbursement of doctors and facilities that are not in the network of your new healthcare company (for a limited time).
- Or, a transition of care can be between Aetna and Included Health as a result of moving in/out of Florida, Georgia, Louisiana or Oklahoma.
If you are transitioning to Aetna effective January 1, or need assistance with a care and disease management case, contact Included Health to discuss eligibility for the transition of care and to obtain assistance completing the transition of care form. Included Health is available 24/7 at 1-833-938-9874 (TTY: 711).
If you want to eat better, be more active or take charge of your health, we can recommend coaching support and personalized health actions that might be a fit for you. But you set the tone, pace and how and when to interact. Aetna lifestyle coaches can help you:
- Identify stress triggers and how to cope with and reduce stress.
- Maintain good eating habits and manage or lose weight.
- Better manage conditions such as high blood pressure, high cholesterol and more.
To access the program, sign in at Aetna.com and select Well-being Resources or get the ActiveHealth® app. You can also call 1-855-741-8020 (TTY:711), 8 AM to 9 PM ET.
WINFertility nurses are available at 1-833-439-1517 (TTY: 711) Monday through Friday 9 AM to 9 PM ET to help you select a high quality provider, understand your treatment options, and provide clinical and emotional support. Family Building benefits have a $35,000 (effective July 1, 2024) medical lifetime limit (enrollment with WINFertility and completion of a nurse consultation is required to unlock this benefit level, otherwise the medical lifetime limit is $10,000).
